·         Microchip number (mandatory)

·         Desexing Certificate

·         Dogs Queensland Membership

·         Proof of Pension Entitlement

·         Proof of Disability/Assistance Dog requirements

·         Toowoomba Dog Obedience Club Certificate

·         ABN/Proof of Primary Producer Status
